If you have LVD hardware version 2 or 6 (type 'head version' to check), then be sure to load LCA bundle lca_bndl.hex. Older versions of the LCA bundle (lcav103.hex, lcav105.hex, etc.) will not function with the hardware. Re-load the correct LCA bundle.
